What is your new patient procedure?

Our office requires a referral to receive treatment. Once your referral is received and intake paperwork is complete, we will get you on our schedule! 

I don’t have my insurance card with me, can I still receive treatment?

We do require a copy of your insurance card for our records before we begin treatment.

How much will my copayment be?

Your co-payment is dependent upon your insurance plan. We will be able to discuss this in detail before your scheduled appointment.
